Principal Bank offers forms for use in servicing your existing Principal Bank IRA. Just complete the form online, print it on your printer, sign and send the form to the bank at the address listed with each form. Select the specific form you need below:
IRA Distribution Form - Use this form to request a distribution from your Principal Bank Traditional, Roth, Safe Harbor or Plan Term IRA. We must receive this form prior to processing any distribution.
IRA Contribution Form - Use this form to tell us which type of IRA, which tax year and which existing Principal Bank account you're contributing to.
IRA Designation or Change of Beneficiary Form - Use this form to designate a new or additional beneficiary for your existing Principal Bank Traditional, Roth, Safe Harbor or Plan Term IRA.
IRA eSignature Card (PDF: 221 KB) - Use this form to complete the new account process when your IRA has been opened electronically. Please note: This form should not be used for Safe Harbor & Plan Term IRAs.
Safe Harbor & Plan Term IRA Signature Card (PDF: 33 KB) - Use this form to 1) acknowledge and accept ownership of the account and 2) certify your Taxpayer Identification Number (Social Security Number).
Note: Receipt of the Safe Harbor & Plan Term IRA signature card is required before transfers, rollovers or distributions can occur on your account.
