Request for Transfer or Conversion to an IRA

This form is used to assist in moving eligible IRA assets from one custodian or trustee to another, or to convert Traditional IRA assets to a Roth IRA. This form should accompany the IRA Enrollment Form.

Notes when completing the form:
  • Failure to complete all necessary information may create a delay in processing
  • Direct transfers: Indicate if the entire balance of the IRA is to be transferred over or only a portion of the plan. If only transferring a specific investment from the other IRA, please include the account number and any maturity information that may impact the redemption.
    Conversions: Indicate if the entire balance of your Traditional IRA is to be converted or only a portion of the IRA. Be sure to complete the Federal Withholding Election section for income tax withholding. If no election is made, a flat rate of 10% will be withheld for federal income tax.
  • If you are age 70 ½ or older, you are required to satisfy the annual Required Minimum Distribution (RMD). The penalty for failure to take the annual RMD for any distribution year is a 50% excise tax on the amount not distributed.
