Safe Harbor and Plan Term IRA Forms
Principal Bank offers forms for use in servicing your existing Principal Bank Safe Harbor or Plan Term IRA. If you would like to make a Cash Distribution, Transfer Funds to an IRA at Another Institution or Rollover Funds to an Employer Sponsored Plan (e.g. 401(k)) please select the appropriate link below. Complete, print, sign and return the forms to the bank at the address provided.
Cash Distribution & Rollover Funds to Employer Sponsored Plan (e.g. 401(k) (PDF: 163 KB) - Use this packet to request a distribution or a rollover distribution.
Note: Please contact your employer to ensure they will accept the funds being rolled into their plan. You will also need to obtain the rollover instructions, which will include information on how to make the check payable and where to send the funds.
Transfer Funds to an IRA at Another Institution (PDF: 1 MB) - Use this packet to transfer funds to an IRA.
Note: To transfer your Safe Harbor or Plan Term IRA to another IRA at another institution, please contact that institution to obtain their IRA transfer paperwork. Once the transfer paperwork is completed and signed by you (as the owner) and signed by a representative from the institution (new custodian); you may send us the transfer form along with your Signature card and AOI.
To access other Safe Harbor or Plan Term IRA forms, go to the Account Login box in the upper left of the screen and select "Personal" from the drop-down menu to login to your account.
- If you already have a "Personal" username and password, proceed with your login.
- If you do not have a "Personal" login, or if you're unsure, go to Establish Your Username and Password.
