Safe Harbor IRA Forms

Principal Bank offers forms for use in servicing your existing Principal Bank Safe Harbor IRA. Just print the forms on your printer, complete, sign and send them to the bank at the address listed with each form. Select the specific form you need below:

Safe Harbor IRA signature card (PDF: 48 KB) - Use this form to 1) acknowledge and accept ownership of the account and 2) certify your Taxpayer Identification Number (Social Security number).
Note: Receipt of the Safe Harbor IRA signature card is required before transfers, rollovers or distributions can occur on your account.

Affidavit of Identity (PDF: 234 KB) - Use this form to authenticate your identity and verify your signature.
Note: The Affidavit of Identity will need to be notarized and witnessed.

IRA Distribution Form - Use this form to request a distribution from your Safe Harbor IRA or to elect to do a direct rollover to a new employer-sponsored plan. We must receive this form prior to processing any distribution.

Form W-9 (PDF: 47.8 KB) - Use this form to certify your Taxpayer Identification number. Receipt of this form may be required before any distributions, transfers or rollovers will be allowed on your Safe Harbor IRA.
