Group Claim Services

Medical Claim Services

• Employees can monitor medical claim activity online by logging in to the Health & Wellness Center. Go to the Claim Activity tab to view and/or print your Explanation of Benefits (EOB).
• Get information about Flexible Spending Accounts (FSA), including benefits and claim forms.

Dental Claim Services

• Principal Life accepts dental claim forms from providers, or if needed an employee can obtain a dental claim form from his/her employer or can access the dental claim form online (PDF: 88 KB) to make filing a dental claim a snap.
• Employees can monitor dental claim activity on the Member Web site by clicking the Claim Activity tab under Health.

Disability Claim Services

• Visit the Disability Claim Management Services section for details on how to file claims, processing timelines, resources used, and much more.
• Access disability claim forms to help make filing a disability claim a snap.
• Employees can monitor disability claim activity on the Member Web site and clicking the Claim Activity tab under Insurance.
• Court of Appeals Benesowitz Decision — New York claimants can access information here relating to the review of previously denied group disability claims because of a pre-existing condition.
