How to Submit a Claim
Most providers will submit claims for their services directly to us on your behalf. We encourage this practice as it ensures the best and most accurate submission of your claim.
Gather the following information
If you are submitting your own claim, no claim form is needed. You will need:
- The provider's itemized statement* and
- A photocopy of the front and back of your ID card so we can identify you and your coverage.

*The itemized statement shows what services and treatments were provided. A receipt showing the amount paid or bill showing the amount due is not sufficient for claim purposes.
