Developing a targeted program
To develop a targeted program, we look at the results from step one. Individuals with the highest risk of chronic conditions are identified based on:
- Cholesterol
- Blood sugar
- Blood pressure
- Weight management
Those identified as high-risk have the opportunity to participate in the intervention program. The program is designed to engage, motivate and support participants toward positive changes.
Providing tools and information
The intervention program provides tools and materials to help participants make the health-related changes they need to improve their health. Participants are provided an online health workbook of risk-specific information containing:
- Educational background information
- Recipes
- Exercise and nutrition charts and logbooks
Additionally, a monthly online newsletter that is specific to the recipient’s condition is available to further inform and raise awareness.
