What is a Qualified High Deductible Health Plan?
A qualified high deductible health plan is a plan that meets minimum deductible requirements and maximum out-of-pocket limits. These requirements and limits are outlined below.
All covered benefits must be subject to the deductible with the exception of preventive care. Preventive care can be subject to co-pays and not subject to the plan deductible.
To contribute to a Health Savings Account, the law requires that the participant must be covered by a qualified high deductible health plan.

Each year, these limits are adjusted based on the cost of living (rounded to the nearest $50), and provisions refer only to the in-network benefits (i.e., out-of-network benefits can be less generous).
