- Self administered drugs must be filled through Caremark. For more information, refer to Caremark’s Specialty Rx phone line at (800) 237-2767.
- Provider administered drugs require precertification.
Modifier JW: Discarded Drugs Not Administered
The following administrative policy is effective March 1, 2010.
