How to Enter

1. First, get all the facts
   • Review the official rules of the program.
   • Get details about the program by reading the frequently asked questions.
   • Find out how the companies are judged.
2. Second, find out what information you will need
   You will be asked detailed questions about your health plan, insurance benefits, pension and retirement plans.
   • Want to gather the information ahead of time? Use the print version of the entry form as a guide (PDF: 177 KB).
3. Finally, fill out the entry form
   • Who can fill out the form? HR directors/office managers/business owners at small and medium-sized businesses may submit this entry form.
   • Submit your entry.
     • Complete the online entry form or
     • Print and fill out the paper version (PDF: 177 KB) of the entry form and mail it to*:
