Then, fill out the entry form

1. Who can fill out the form? HR directors/office managers/business owners at small and medium-sized businesses may submit this entry form.
2. Submit your entry either by:
• Completing the online entry form or
• Printing and filling out the paper version (PDF: 177 KB) of the entry form and mail it to*:
