Benefit Continuation - Blue Valley Unified School District

As a terminating participant in the Voluntary Term Life and Long-Term Disability programs provided by Blue Valley Unified School District and underwritten by Principal Life Insurance Company, you may have options for continuing coverage.

  • Portability of Life Insurance - Extend or "port" your current coverage, without any medical questions.
  • Conversion of Life Insurance - Change or "convert" your coverage to individual universal life insurance, without any medical questions.
  • Conversion of Long-Term Disability Insurance - Change or "convert" your coverage to a group long-term disability insurance policy issued by Principal Life.
  • Dental Discount Plan - Get immediate access to dental savings for anyone, at any age.

Life Coverage During Disability (premium waiver)
In the event your coverage is terminated (as a result of a disabling condition) and you are subsequently approved for Life Coverage During Disability (premium waiver), your converted and/or ported coverages will be terminated and any premiums paid will be refunded to you.

If you participate in the Voluntary Term Life insurance program, you have the option of porting the coverage in lieu of conversion. You select the amount of coverage you wish to maintain. Proof of good health is not required unless you elect to increase your coverage amount. The minimum amount of coverage you may continue is $10,000.

You may also elect to continue coverage for any covered dependents if you continue your coverage. Proof of good health is not required unless you elect to increase a dependent's coverage amount.

To continue this insurance coverage, complete the Portability Application. It's not necessary for you or your former employer to complete the "Employer to Complete" section, as that information has already been provided to the insurance carrier.

The portability rates are included on the Rates document.

Please refer to your certificate of coverage regarding any limitations and termination provisions for this coverage. If you no longer have a copy of this, one will be provided upon your request. You must apply for this coverage within 60 days of losing coverage.

After completing the application, send it and your check for the first month's premium to:

You'll then receive a letter from Principal Life explaining your coverage amounts and benefits as well as when and how to make future direct premium payments.

Individual Purchase Rights (conversion) allow qualifying individuals previously covered under Voluntary Term Life policies, to buy individual life insurance coverage without proof of good health. Coverage is provided through an individual Life Insurance policy issued by Principal Life Insurance Company and Principal National Life Insurance (except in NY). Dependents may also convert their Voluntary Term Life coverage independent of the employee's conversion. The minimum amount of coverage you may convert is $1,000 to the maximum amount of insurance you had under your former employer's policy.

To continue this insurance coverage, complete the Conversion Application. Please refer to the Instructions & Rates for assistance in completing the application. It's not necessary for you or your former employer to complete the "Employer to Complete" section, as that information has already been provided to the insurance carrier.

Premium rates can be found on page 3 of the instructions document. You must apply for this coverage within 31 days of losing coverage.

Important note: If you are currently receiving long-term disability benefits, you are not eligible for LTD conversion.
For employees enrolled in your former employer's LTD coverage, you may convert your coverage if it is terminated for any of the following reasons:

  • You resign.
  • You are terminated for cause.
  • You are laid off.
  • You go on a leave of absence.

Eligibility

  • You must have been covered at least 12 consecutive months under your former employer's policy prior to coverage terminating.
  • These 12 months can be a combination of coverage under Principal Life's group policy or prior group LTD coverage.

Converted LTD coverage is continued under a group long-term disability insurance policy issued by Principal Life. Premiums are based on your age and benefit level.

Any benefit payable is based on your pre-disability monthly income. This was defined in your former employer's group disability policy. The maximum benefit during the disability benefit period under your conversion policy will be the smallest of: a) $4,000/ month; b) 60% of your conversion earnings; c) the maximum benefit under your former employer's group disability policy.

Your benefit is reduced by other income you receive, such as primary or family Social Security benefits, Workers' Compensation benefits, and sick pay or salary continuance. Refer to the Benefit Summary for a more inclusive list of other income sources.

To continue this insurance coverage, complete the LTD Conversion Application. The Employer information has already been provided to the insurance carrier. Premium rates are included in the Benefit Summary. You must apply for this coverage within 31 days (45 days for New York residents) of losing LTD coverage.
