How to change office information
If you need to update your office information, follow these 2 steps:
Step 1: Complete record update form (PDF) and, if needed, a W9 (PDF).
Step 2: Return completed form by email or fax:

How to add a location
Adding a new location for an existing provider is easy. If the location is in:
The same state and uses same TIN: Complete and return the additional location request form (PDF) and proof of malpractice coverage.
A different state and/or uses a different TIN: Contact us at DLGRPPFGPROVIDER@exchange.principal.com. You may also need a W9 (PDF).
How to re-credential
After you’re one of our providers, we’ll periodically re-credential you. In most cases, we can do this without needing any additional information from you. If we do need more information, we’ll call or send a letter requesting this.
Utilization review and quality assessment program
To make sure our members are getting quality care, we may periodically review your treatment records. These reviews will determine if your services and record keeping practices are consistent with the methods and procedures established by Principal®. All network providers agree to participate in and accept the decisions resulting from the utilization review and quality assessment program. Your participation in our network may be declined or terminated based on the findings from these reviews.
